Self Determination Program Transitional Period Consulting (Copy)
Congratulations! You’ve made the decision to transition yourself or a family member into the Self Determination Program (SDP). The Person Centered Plan (PCP) is the first step in the process, but what comes after submitting the PCP? The transitional period between submitting the PCP and officially enrolling in SDP can be overwhelming and daunting… and that is why we’re here to help. It is important to note that the transitional period can take anywhere from 4-7 months. The consulting fee is only charged once no matter how long the transitional period lasts and all responsibilities are upheld until the participant’s official start date. The transitional package includes the following support:
Conduct regular consultations to address all concerns, comments, and questions that SDP participant and their family may have
Reviewing existing IPP to ensure it is up to date
Assist in developing an Individual Budget (IB)
Advocate for participant’s unmet needs
Responsible for creating acceptable Spending Plan (SP)
Provide guidance on SDP to participant and family
Assist participant and their family with their Individual Program Plan (IPP) if needed
Coordinate how services and supports are provided to participant
Assist participant and FMS onboard staff
Meet with potential vendors and employees that participant is interested in obtaining services from
Create and deliver detailed list of vendors/ employees to the participant
Educate family on SDP information and generic resources
Create budgeting sheets
Act as liaison between participant, RC, and FMS to ensure efficient delivery of important documents and communication
Our ultimate goal is to help you or your loved on have a great experience when embarking on this exciting new journey. We recognize that some people may need financial assistance and that is why we are more than happy to offer payment plans of 2, 3, or 4 months.
